Online Referral

You do not need a referral to see an Accredited Practising Dietitian. However, a referral allows good communication between your doctor/specialist and your dietitian on your health journey. You may also be eligible for a health care plan in which a referral is required by your GP and it will provide you benefits through Medicare.

Note to GP’s: Medicare Health Care Plans during COVID are eligible for bulk billing

Health Professionals may use the form below to serve as a referral. Please note that any relevant documents or care plans will still need to be provided to the patient or fax to (02) 9614 5919.
