Online Referral

You do not need a referral to see an Accredited Practising Dietitian. However, a referral allows good communication between your doctor/specialist and your dietitian on your health journey. You may also be eligible for a health care plan in which a referral is required by your GP and it will provide you benefits through Medicare.

Health Professionals may use the form below to serve as a referral. Please note that any relevant documents or care plans will still need to be provided to the patient or mailed to Suite 19, Westmead Private Hospital,  Corner Mons and Darcy Roads, Westmead 2145.
